TOLEDO, Ohio — The presiding judge in the DePuy (NYSE: JNJ) ASR hip implant multidistrict proceedings has approved a mandatory supplemental disclosure form for plaintiffs that solicits specific information on bilateral implants and revision surgeries, and requests medical authorizations.

The Case Management Order #15 was entered by Judge David A. Katz in the U.S. District Court for the Northern District of Ohio March 4.

Judge Katz emphasized that the ASR Supplemental Plaintiff Disclosure Form is “a separate and independent form and must be completed” by all plaintiffs, even if they have already filed Preliminary Disclosure Forms pursuant to CMO #5 …
